    Died while responding to a floundering vessel off the Hawaiian Islands, the HH052 crashed in deteriorating visibility into a cliff, killing all aboard. (Download pdf)

    Killed during Marine Safety Inspection on March 3, 1993

    LCDR Turek was the lead marine inspector aboard M/V Cape Diamond. During a test of the engine room fire extinguishing system, a system malfunction caused a prolonged release of carbon dioxide into the space. LCDR Turek perished when he entered the engine room to warn others to escape. (Download pdf)

    Killed at Bates Field in Mobile, Alabama. While on a training mission, his helicopter lost a pitch control rod and immediately crashed thereafter. (Download pdf)

    In a narrow channel meeting between the Capricorn and the Blackthorn, the two vessels collided, capsizing, and sinking the Blackthorn. ENS Sarna dove in to the water to rescue a trapped crewman, and finally on one of the dives, failed to surface. (Download pdf)
